Саҳми омилҳои муҳити зист дар бемории Паркинсон

Пас аз ба ҳисоб гирифтани фарқиятҳои синну сол, ҷинс ва кишвар, истеъмоли ҳарҳафтаинаи caffeine ва истифодаи tobacco, ки ҳадди ақал панҷ сол идома кардаанд, бо Parkinson disease робитаи баръакс нишон доданд. Limitations of the Study

General Conclusion

LARGE-PD research presents an important study evaluating environmental factors associated with Parkinson disease together in a large Latin American sample. Findings show positive associations of head trauma, rural living, well-water consumption, occupational pesticide exposure and metal exposure with Parkinson disease.

Joint population attributable fraction of these five risk factors was calculated as %13,7 in the main analysis. Attributable fraction appeared higher in men and participants under 60 years, while large differences were found between countries.

Although caffeine and tobacco use showed inverse associations with Parkinson disease, these findings are not evidence of causal protection. Tobacco use in particular cannot in any way be interpreted as a prevention method because of its severe and proven health harms.

The most important message of the study is that the environmental dimension of Parkinson disease needs to be investigated more systematically in Latin America. Drinking-water safety, occupational toxic exposures and prevention of head trauma are not merely individual-behaviour issues; they are structural areas requiring environmental, occupational-health and public-health policies.

Research was conducted using multicentre observational case-control data from the LARGE-PD consortium. The analysis included 5.729 Parkinson patients and 4.158 controls from nine Latin American countries.

Parkinson diagnosis was determined according to UK Queen Square Brain Bank criteria. Participants’ demographic characteristics, family history, cognitive assessments, disease duration in Parkinson patients and clinical stages were recorded.

Environmental exposures were assessed with a standard questionnaire asking participants about lifetime experiences. Exposures were modelled as at least five years, each additional ten-year duration, any lifetime exposure, and total intensity where suitable.

Categorical variables between groups were compared using chi-square tests, and continuous variables using t tests or Mann-Whitney U tests depending on distribution. The association of each environmental exposure with Parkinson disease was analysed using logistic regression.

Main multivariable models were adjusted for:

  • Age,
  • Sex,
  • Country.

Benjamini-Hochberg false discovery rate correction was applied to reduce false-positive results from multiple comparisons. Statistical significance threshold was set at P ≤ 0,05.

Tea, caffeinated soft drinks, alcohol, non-occupational pesticide exposure and living near a farm did not show significant associations with Parkinson disease in the main adjusted analyses.

Five risk factors were included in the PAF analysis and the joint fraction was calculated as %13,7. Individual fractions were %6,5 for head trauma, %6,0 for rural living, %4,2 for well water, %2,5 for occupational pesticide exposure and %1,1 for occupational metal exposure.

At country level, highest joint PAF was calculated as %31,5 in El Salvador, %26,9 in Brazil and %20,8 in Colombia. Joint PAF was %23,2 in men and %6,6 in women. Across age groups, highest value was %19,4 in the 50–60 years group.

When years of education were added as an adjustment variable, joint PAF fell to %10,8; when family history and tobacco use were also added, it was calculated as %10,5.
